To begin the intake process, please complete our brief online intake and consent forms linked below. If using insurance, the following documentation will be needed:
Physician’s order for the AAC evaluation (we can request this once intake and consents are complete).
Recent medical visit notes from your child’s physician or PCP, dated within six months of the device order, documenting the need for an AAC device.
DDD Person-Centered Service Plan (PCSP), if applicable (we can request this with your support coordinator’s contact information).
Previous evaluations, therapy progress reports, and relevant medical or school records are also helpful in supporting the future device funding request.
